Primary hyperparathyroidism occurs in 1 per 1000 persons in the United States. Individuals typically have four parathyroid glands that are located at the periphery of the thyroid gland in the lower neck. These glands regulate serum calcium and phosphorous levels. Parathyroid disease is characterized, and most often diagnosed, when a serum calcium level is persistently elevated along with an inappropriately elevated serum parathyroid hormone (PTH) level. The symptoms that manifest from primary hyperparathyroidism are kidney stones, abdominal pain with constipation, mood changes (general fogginess), and bone loss with easy fractures. In 85% of the cases, a single abnormal gland (adenoma) is the culprit. However some people can have more than one adenoma or four gland hyperplasia. The diseased and enlarged gland can often be localized with an ultrasound or nuclear medicine scan. The treatment for primary hyperparathyroidism is surgical removal of the abnormal gland which is often a brief surgery. Secondary and tertiary hyperparathyroidism is less common and occurs most often in patients with chronic renal disease. Cancer of the parathyroid glands is extremely rare.
