Thyroid abnormalities are common and are best evaluated with laboratory testing and ultrasound evaluation when there is suspected disease. Prevalence of thyroid disease is more common in females for unknown reasons. Palpable thyroid nodules occur in 10% of the general population in the United States. Roughly 95% of these nodules are benign but typically require tissue sample with ultrasound guided fine needle aspiration (FNA) to rule out thyroid cancer. Risk factors for thyroid cancer include radiation exposure and a positive family history but the majority of thyroid cancer is sporadic. Thyroid cancer is one of the few cancers that is increasing in incidence but also has a very high cure rate (>95%). Treatment for thyroid cancer typically involves removing the thyroid gland and occasionally post-operative radioactive iodine therapy.
Disorders of the thyroid can result in underactive (hypothyroidism) or overactive (hyperthyroidism) glands that may need to be regulated with medication. Serial laboratory testing and medical therapy are often needed to correct the hormone imbalance. The thyroid gland controls our metabolism, e.g. weight, temperature, hair and skin health. Some individuals develop enlargement of the gland (goiter) in the face of multiple nodules. This condition can be monitored with ultrasound and labs and if compressive symptoms develop possible thyroidectomy.
